Add a Respondent (Employee) to your Survey

Add an employee to your survey

To add an employee to your survey, you will need to complete a request form.
The form requires you to provide the following information - we recommend gathering it together before you begin completing the form:
  1. Employee Number/Employee ID
  2. First and Last names as they appear in your payroll or HRIS system
  3. Position Title/Job Title as it appears in your payroll or HRIS system
  4. Email address
  5. Date of hire
  6. Year of birth (do not provide the employee's full date of birth)
  7. Gender
  8. The name of the department, location or business unit
  9. The code of your department, location or business unit - if your company assigns one. If you do not have one, enter "none"

Only a manager or your company's project administrator may add an employee to a survey.
Invitation requests must be approved by your company's project administrator.
Only one invitation can be issued to an employee's email address. If your employee cannot find their invitation, please refer to this article to help them find their invitation.
